We are excited you have chosen The College at Brockport and look forward to your arrival on campus. As a potential student-athlete there are a number of prerequisites you need to attend to
prior to your arrival. Please read this document carefully and follow all directions as outlined.
No student will be allowed to participate in any way until all forms are complete and on file. In order for you to be eligible for intercollegiate athletic participation there are a number of forms and steps you need to follow.
All health, insurance, and verification forms are found on-line at the Health Center Portal-MY HAZEN
*In order to register for the portal, you must have paid your deposit.*
MY HAZEN works best in Chrome browser. Go to Student Health Center and click MY HAZEN to sign in using your NetID and password. This is the same login/password used for Banner, and email.

There are four sections to the New Student-Athlete compliance process. You must complete all four to be cleared to tryout for a team.
Four sections of NCAA Compliance: Health Center, Athletic Training, Eligibility & Title IX compliance.
Section One: Health Center
The National Collegiate Athletic Association (NCAA) mandates that all potential student-athletes, both freshman and transfers, have a current medical evaluation form on file prior to participation in intercollegiate athletics. On the
MY HAZEN you will see the following forms and steps to complete:
- New NCAA Athlete Health History form
ONCE YOU BEGIN THIS FORM YOU MUST COMPLETE IT. PLEASE allot at least 20 MINUTES to do so. INCOMPLETE FORMS WILL NOT BE PROCESSED BY THE SYSTEM AND CANNOT BE SAVED. The system will not allow you to submit your form unless all of the questions are complete. Once you have completed the form, click submit.
- Physical Form for your primary doctor
THE PHYSICAL MUST HAVE BEEN PERFORMED WITHIN THE LAST 6 MONTHS. Download this form and take it to your doctor to be completed.
- Upload completed physical form and immunizations to the MY HAZEN site.
4. Student-athletes who wish to participate in intercollegiate athletics will be
individually responsible for all the costs associated with injury or illness resulting from involvement in athletics. Every student-athlete must have proof of health insurance that provides coverage for athletic-related injuries up to $90,000. A copy of your medical insurance card, front and back, will need to be on file prior to your participation
Section Two: Athletic Training
On MY HAZEN
- Read and sign off on Concussion Education form
- Complete Consent for Release of Information form
- Complete Injury and Illness Acknowledgement Form
- Provide Sickle Cell Trait test results or Complete waiver
- Complete verification of Insurance form
Section Three: NCAA Eligibility
NEW STUDENT-ATHLETE FORM
- All student-athletes need to complete the appropriate pages
2. PRINT THE CONFIRMATION EMAIL received after the NCAA eligibility form is submitted
3. BRING it to the Eligibility meeting
Section Four: Title IX Training
All new students and student-athletes are required by the SUNY Chancellor to participate in orientation and continuing awareness programs to educate about preventing sexual violence, how to report it, and what resources and remedies are available for victims/survivors. To address this mandate, all new student-athletes must complete the online training "Think about it". This program takes 2 ½ hours to complete and can be broken into sections.

The following deadlines have been established for each sports season. If you do not complete all of the documents by the deadline you will not be able to start practice on the first day.
