We hope that you are having a wonderful summer and training hard for your upcoming season.  As we prepare for the new season, it is time to start filling out the compliance forms.
 
In order to make the process easier for you, I have outlined the steps and forms you need to complete.  When you log on to MyHazen there is a section specifically for returning student-athletes.  PLEASE complete the correct section and follow directions.   Filling out unnecessary forms creates more work for everyone.
 
Fall student-athletes MUST HAVE THEIR PAPERWORK COMPLETED BY AUGUST 1 OR THEY WILL NOT BE ABLE TO COME IN FOR PRESEASON CAMP.

STEP BY STEP DIRECTIONS FOR NCAA COMPLIANCE
 
Four sections of NCAA Compliance: Health Center, Athletic Training, Eligibility & Title IX compliance.
 
Section One: Health Center
On the MY HAZEN home page:
 
  1. Complete ANNUAL UPDATE FOR RETURNING ATHLETES form
  2. Student-athletes who wish to participate in intercollegiate athletics will be individually responsible for all the costs associated with injury or illness resulting from involvement in athletics. Every student-athlete must have proof of health insurance that provides coverage for athletic-related injuries up to $90,000.
You DO NOT have to upload your insurance cards unless your insurance company has changed since last year.
     
Section Two: Athletic Training
 On the MY HAZEN home page:
 
  1. Read and sign off on Concussion Returning Athlete form
  2. Complete Consent Returning Athlete form
  3. Complete Injury and Illness Returning Athlete Form
  4. Complete Insurance Returning Athlete
 
Section Three: NCAA Eligibility:
RETURNING STUDENT-ATHLETE FORM 2018-2019
 
  1. All student-athletes need to complete the appropriate forms.
  2.    PRINT THE CONFIRMATION EMAIL received after the form was submitted
  3.    BRING it to the Eligibility meeting
 
Section Four: Title IX Training:
Mandatory Title IX trainings is an annual requirement for all student-athletes.
 
All returners will have their training during the eligibility meeting.   The training will last approximately one hour.
 
 
The following deadlines are for each sports season.  If you do not complete all of the documents by the deadline you will not be able to start practice on the first day.
 
Fall teams – August 1,  Winter teams – August 30, Spring teams – August 15
