Forms for New/Transfer Student-Athletes

Forms for 2020-2021 Academic/ Athletic Year will be available July 1, 2020.

By NCAA, and our College standards, all new athletes are required to receive a physical exam BEFORE athletic participation. Take the College at Brockport Athletic Physical Exam Form with you to a medical doctor for him/her to fill out and sign.

1. Upload to MyHazen.

 
Please fill out the following forms on the MyHazen:

1. Health History
2. Physical Exam
3. Sickle Cell Trait Form
4. Insurance Verification
5. Consent of Release of Information
6. Concussion History and Baseline
7. Concussion Education
   • Concussion Management Plan
   • Concussion Education Video
   • Concussion Fact Sheet
8. Injury/Illness Acknowledgment Form

 
In MyHazen, provide health insurance information, upload front and back copy of insurance cards (No PDFs).
 
Complete and return all forms by August 1
 

Forms for Returning Student-Athletes

By NCAA, and our College standards, all returning student-athletes are required update health history and forms before athletic participation.
 
Please fill out the following forms on the MyHazen:

1. Update Health History
2. Insurance Verification
3. Consent of Release of Information
4. Updated Concussion History and Baseline
5. Concussion Education
   • Concussion Management Plan
   • Concussion Education Video
   • Concussion Fact Sheet
6. Injury/Illness Acknowledgment Form
